REDUCED FEE
Reduced fee services are available on a limited and as-needed basis.
INSURANCE
I accept some of the major insurances available. These include:
- Aetna
- Anthem BlueCross BlueShield
- Cigna
- CT Husky Program (Medicaid)
PAYMENT
Checks, PayPal, Venmo, and Debit/Credit Cards are accepted for payment at the time services are provided. If you consistently fail to pay your co-pay or session fee, I may suspend our meetings until you are current with payment. Also, there is a $20 fee for any returned checks. If it occurs more than one time, I will only be able to accept PayPal or Venmo for our sessions.
CANCELLATION POLICY
I cannot bill insurance companies for missed appointments. If you are unable to make an appointment, please give at least 24 hours notice by calling my office or emailing me at jsslmft@gmail.com. I will make every effort to contact you within one business day to reschedule. If late cancellations or missed appointments occur more than one time, except in the case of a personal or family emergency, you will be charged for the time reserved.
